Don't assume piriformis
Screen neuro signs and work away from the sciatic notch. Refer on progressive weakness, saddle numbness, or bladder or bowel change.
Deep gluteal and lateral hip pain reward testing over assumption. This lesson maps deep gluteal, sciatic interface and lateral hip load patterns, and retests under load.
For buttock pain that is worse sitting, test the hip, the hamstring origin and the sciatic interface rather than assuming piriformis. For lateral hip load, avoid digging the insertion. Treat the surrounding compression and retest stance. Work away from the sciatic notch.
